<p>Until 2002, in the ED of Children&#8217;s medical Center in Dallas, we sometimes allowed families to be present at patient&#8217;s bedsides during invasive procedures (IPs, such as central line placement, lumbar puncture, and chest tube insertion) and resuscitation interventions (RIs, including emergency endotracheal intubation and cardiopulmonary resuscitation). However, we had no guidelines or policies on the presence of family-defined as relatives or significant others with whom the patient shares an established relationship-during such interventions, nor did we have any formal support from our ED administartors. In practice, we drew the line on certain procedures that we deemed too painful fro families to witness.</p>
<p>In 1995 the Emergency Nurses Association (ENA) developed clinical guidelines to support the option of family presence during IPs and RIs Other health organizations, including the American Heart Association (AHA) and the American Association of critical-Care Nurses, have since followed suit. In addition, advanced training programs such as the AHA&#8217;s Advanced Cardiac Life support Course and Pediatric Advance Life Support Course and the ENA&#8217;s Trauma nursing Core Course and Emergency Nursing Pediatric Course have been incorporating family presence recommendations into their curricula.</p>
<p>we decide that our ED should have a written policy regarding family presence. First we examined recent surveys, many of which found that the majority of family members want to be in the room while their loved ones are undergoing emergency procedures.</p>

<li>Understand      that your love one will grieve over the loss like any other, and is likely      to progress through similar stages of denial, anger, sadness and      ultimately, acceptance. Until she gets to acceptance, there is little you      can do beyond just being there.</li>
<li>Spread      the word about your loved one’s loss tactfully among close family and      friends. Your loved one may be reluctant or unsure how to do this, but      it’s important for others to know so they can provide valuable help and      support.</li>
<li>encourage      your loved one to stay involved with family and friends, professional      associations, hobbies, etc. Don’t let her embarrassment about the layoff      lead to unhealthy isolation.</li>
<li>Make      sure your love one meets any deadlines for continuing health insurance      coverage and/or other equally important concerns.</li>
<li>Offer      to help in any way you can- and then back off. You may be understandably      concerned about family finances, but nagging and pressuring someone who      likely is already pressuring herself far more is unproductive, at best.</li>
<li>Be      alert for signs of depression. Eating, drinking or sleeping too much or      too little, along with a consistently depressed mood, may be signs she      needs more help than you can provide.</li>

			<content:encoded><![CDATA[<p>Nancy Ulene 43, wasn’t particularly worried when a routine mammogram turned up something her radiologists thought was fishy. She had a tumor seven years earlier that turned out to be benign. But this time was different. A biopsy confirmed that Ulene, the niece of former Today Show medical expert Art Ulene, had ducal carcinoma in situ, or DCIS, a growth that is variously described as either an early-stage- breast cancer or a precancerous lesion. “It was very confusing” says Ulene, a color stylist for Walt Disney TV Animation “I needed to know more.”</p>
<p>What she soon learned was that kind of cancer she had- a group of malignancies so tiny that they were rarely seen before the advent of mammograms powerful enough to spot them- is at the heart of raging debate in the cancer community. Doctors know what to do when they find tumors the size of marbles or plums. That’s what surgery, radiation and chemotherapy are for. But what do you do with cancers the size of pencil points? Do you treat them alone? Should you even be looking for them in the first place?</p>
<p>This year, according to the American Cancer Society, some 200,000 women (and 1,500 men) will learn that they have breast cancer- up from a little more than 100,000 two decades ago. While the death rate from the disease has dropped modesty over the past decade, there is a growing sense of frustration among cancer experts. Part of the problem is DCIS. Thirty years ago, these miniature tumors, which usually don’t spread into the rest of the body, were diagnosed in some 6% of breast cancer patients. Today the ratio is closer to 20%, largely because of advances in detection techniques. Yet the treatment of choice is still surgery followed by radiation. “We may be far over treating our patients” says Dr. Julie Gralow, an oncologist at the Fred Hutchinson Cancer Research Center in Seattle. “We’ve now got women being diagnosed with tumors that probably never would have been treated if we didn’t have mammography. They probably would have lived long, natural, healthy lives never knowing they had breast cancer.</p>
